Whether you are recuperating from an illness, or coping with a long term medical condition, we are here to help. 

 

Our services are directed by the patient’s physician and supervised by an RN.  To ensure you get the proper care, we meet with your doctor to determine your needs and we always follow doctor’s orders.

RN supervised care provides the expert care we insist for our patients.  A primary nurse, who acts as the patient's case manager, assists in designing a care plan to meet individual unique health care needs. The RN coordinates and supervises services and care providers.

 


 

Nurses

Our nurses provide direct care, education and patient assessments. Basic skilled services may include:

  • patient assessment

  • direct nursing care

  • patient/family education

  • supervision of assistive staff

  • skilled care of ventilators or ostomy care

  • basic nursing care monitoring vital signs and medication administration

  • Our nurses have received the education and training as well as experience in home health care to ensure their competence.

     


     

    Therapists

    Physical, Occupational and Speech Therapists are available as needed to complete the health care plan. Other services can be made available as required.

     


     

    Home Health Aides

    Home health aides supplement nursing and therapeutic care. They can visit on a daily or as needed basis.  Supervised by a RN, the Home Health Aide can:

  • provide personal care assistance, hygiene and personal grooming [shampoo, oral hygiene, hair, nails]

  • simple meal preparation

  • assist with exercise

  • provide limited housekeeping

  • If directed by patients' physicians, additional services may be requested.

     


     

    Support Staff

    For times when a patient needs more constant care than a home health aide can provide, we have homemakers and companions available.
