What We Offer
Continuous Skilled Nursing
Also known as private duty nursing and block time nursing, CSN program offers nursing services in the home. Our licensed professionals are patient-centered and care-focused. We offer skilled patient assessments, direct nursing care, patient/family education, and supervision of care providers for medically fragile pediatric and adult populations.
Skilled Intermittent Care
Skilled Intermittent Care, also known as a Skilled Visit, is a specialized type of nursing service for our pediatric and older patients. Skilled Intermittent Care is typically less than 1 hour with a frequency of 1-2 visits per day or week. These visits could consist of a nurse visit, physical therapy, occupational therapy, speech therapy, medical social work, or home health aide. Our licensed professionals collaborate with the health care team to achieve the patient’s goal.
Home Health Aide
Our certified home health aide staff assist with basic care needs and home living. Under the direction and supervision of our professional staff, home health aides provide personal care assistance, simple meal preparation, exercise, and light housekeeping. Home health aides visit daily or as needed.
Physical Therapy
Our physical therapists focus on restoration, maintenance, and promotion of optimal physical functions. Physical therapy helps infants born with musculoskeletal birth defects and adults suffering from acquired injuries, recovering from surgeries, and dealing with post-stroke conditions. Our physical therapists use exercises, equipment, and education to help achieve health goals.
Occupational Therapy
Our occupational therapists focus on the client’s ability to do their daily life activities through age-appropriate learning methods. Occupational therapy might include play therapy or teaching a patient to dress or feed themselves. Developmental activities help develop fine motor, sensory integration, visual motor tasks, or gross motor play.
Speech Therapy
Our speech therapists focus on the client’s communication skills, fluency disorders, and swallowing disorders. Speech therapy can help children who are having difficulties expressing or comprehending words or written language. Our therapists will develop a plan to meet communication, language, oral motor, and feeding goals.
School Nursing
Our care coordinators work in collaboration with the school systems to meet both the client's and school’s needs. We provide licensed, trained, and qualified 1:1 nursing staff in a school setting from school-aged to college level.
Maternal Nursing
Our skilled nursing professionals visit after you return home from the hospital to check in on you and your newborn. Our nursing staff during these visits provide support, offer consultation, and help achieve your care plan.
