The purpose of this checklist is to help identify learning needs, provide the learning experience necessary to develop competency, and insure expertise in certain areas. Review the skills list and assess your abilities for each skill. If you have performed the skill and can accurately implement the technique or procedure, check the column "I feel competent performing." If you have never performed a skill, check the column "I have never done." Any skills you have never done or feel you cannot perform accurately should be identified.

Please note that a HHA should never perform any task that has not been assigned by the Primary Nurse. If the HHA is asked by the patient or caregivers to provide a service that is not on the assignment sheet, the HHA will need to contact the Primary Nurse, Supervisor, or Clinical Director.
